Documents Required for Registration

Documents required for registration to be completed:

• Most recent Standardized Test Scores     

• Report card (s) from last/current school — required if student will be in 1st through 8th  

• Birth Certificate

• Baptismal Certificate and First Communion Certificate if applicable

• Health & Immunizations Records

Vision records are required for the following:

• New students to Illinois

• Kindergarten     

Dental Records are required for the following:

• Kindergarten

• 2nd Grade

• 6th Grade

Physicals are required for the following:

• New Students

• Pre-K

• Kindergarten

• 6th Grade
